About Us
Evolve Healthcare Group is a mobile Occupational Therapy service that caters to NDIS and My Aged Care (MAC) participants across Melbourne Victoria, and via Telehealth Australia wide.
Our senior Occupational Therapy team specialise in Occupational Therapy assessment services for both adult and paediatric age groups, including:
Functional Capacity Assessment
Home and Living Assessments (SIL, SDA, ILO)
Assistive Technology Prescription
Home Modifications
Home Care Package Assessments
NDIS Access Assessments
We have extensive experience in navigating NDIS and MAC funding frameworks, and through relentless advocacy, we strive to get the required outcomes for individual needs and goals.
Whilst assessment services are our specialty, we also provide ongoing therapy and capacity building services to participants 15 years and older, with a focus on developing confidence, independence and quality of life.
Founder & Director - Taylah Godeassi
Evolve Healthcare Group was founded by Senior Occupational Therapist, Taylah Godeassi, who has extensive experience in NDIS and My Aged Care sectors. Taylah has a background in clinical leadership - supporting the clinical governance of a large team of Occupational Therapists. Her key strengths lie in OT assessment services, being known for supporting complex funding approvals and being a strong advocate for her clients and their families.
Whilst Taylah’s passion areas are Assistive Technology and Home and Living Assessments, she also finds great fulfilment in working collaboratively with individuals to support ongoing capacity building, improve quality of life and self-efficacy. This includes supporting the aging population to maintain their independence, safety and confidence at home and in the community.
